Addiction Supportive Housing Program
Affordable housing includes assessments, comprehensive and ongoing treatment, case management and addiction counselling, referrals to treatment, and independent life skills training and support

Housing First Program
Case management and individualized treatment plan, help to navigate housing market and secure housing, social and community integration support, counselling, referral to primary care, employment, education, or substance use treatment

Application
Eligibility
Housing First: individuals 16 years and older, who are homeless or precariously housed, and experiencing problems with drugs and/or alcohol, and mental illness without formal diagnosis; individuals will not be excluded for continued substances use, not taking medication, or involvement in criminal justice system
Residency Requirements: Addiction Supportive Housing - York Region and South Simcoe; Housing First - York Region, South Simcoe and parts of Toronto (contact for information)
