Focuses on moving people who are chronically homeless as rapidly as possible from the street or emergency shelters into permanent housing with supports that vary according to participant needs.

- Complete a Common Intake and Consent form
- Complete a common assessment (VI SPDAT)
- Have your request added to the By-Name List
Eligibility
Available to people who:
- Are currently experiencing homelessness
- Meets the definition of chronic homelessness
- Have been assessed (VISPDAT) having a high level of acuity
- Lives in Simcoe County
- Has been added to the By-Name List
