Mobile outreach services from an Advanced Care Paramedic and Social Worker working with community partners to deliver services and help individuals connect with community supports
Advanced Care Paramedic provides
- assessments, triage and interventions, including blood glucose testing for diabetic clients
- medical counselling, and referrals for medical care
- wound care treatment
- clinical support as well as anti-psychotic injections
- virtual medical support if required and other supports for physical and mental health
Social Worker provides
- mental health assessment and intervention for persons experiencing homelessness
- psychological, emotional, and addictions counselling, and other supports for mental health
- service navigation and referrals to community supports, such as addiction and mental health agencies, and counselling for crises, depression substance abuse, thoughts of suicide, trauma, and other mental health concerns

Marginalized populations living in Durham Region, including people who are experiencing homelessness, are under-housed or newly housed, and who may have mental health and addiction challenges as well as medical care needs in the community.
