Waterloo Wellington Home & Community Care Support Services is the single point of access for Supportive Housing / Assisted Living programs. Residents, family members, and health care providers may call directly to make a referral, 519-823-2550 or 1-888-883-3313.

Eligibility
Attendant Services: adults with a permanent physical disability, 16 years and older ; must have valid Ontario health card ; require personal care to live independently ; able to direct own care ; medically stable.
Home Help: older adults 55+ and persons with a physical disability who require assistance to maintain their homes and remain independent, but do not have someone in the household who can assist them.
Ages: 16 years and up
