Oxford County Community Health Centre - Mobile Health Outreach Bus

MHOB has an outreach worker, community partners and scheduled primary care (RN/NP) on board to provide:

  • Care for health concerns
  • Wound care
  • Access to medical referrals
  • Support with form completion
  • Community resources/referrals
  • Harm reduction supplies
  • Personal hygiene supplies
  • Emotional support
  • Basic need supplies

Application

Must attend a bus location.

Eligibility

MHOB can be accessed by anyone in Oxford County who may be vulnerable, at risk, or in need of healthcare or outreach.
