This ServiceOntario site provides the following services:

Driver and Vehicle Services
  • Accessible parking permits - apply, renew, replace or change information
  • Driver's licence - change address, change name, renew licence, replace licence, licence reinstatement, out of province driver's license renewals and temporary driver's licenses

Health Card Services

Ontario Photo Card
  • obtain, renew or replace the Ontario Photo Card
  • change the address, the digital signature and /or name on an Ontario Photo Card

An Ontario photo card is a government-issued photo identification that will allow non-drivers aged 16 years and over to access government, financial or business services that require proof of identity, such as cashing a cheque, opening a new bank account, obtaining a credit card, boarding a domestic flight, or obtaining hotel accommodations. Ontario residents 16 years of age and over who do not hold a driver's licence are eligible for the Ontario photo card.
