Medical care team focused on family and community health that coordinates patient care in order to assess and treat injuries or illness. Team includes family doctors, nurses, nurse practitioners, social worker, family counselor, psychologist, dietitian, diabetes educator, and health promoter. Seasonal flu vaccine by appointment.
