Services
  • primary health care; foot care and chiropody; physiotherapy
  • prenatal education and care including midwifery
  • dental clinic for government funded dental programs for children, youth and seniors
  • health promotion and wellness workshops
  • counselling for registered clients on mental health conditions, relationship issues, substance use, depression, grief and trauma and other challenges
  • sexual health testing, counselling and treatment of sexually transmitted infections (STIs)
  • diabetes education
  • seniors services including wellness workshops, gentle exercise and social interaction
  • youth camp for children and youth 8-16 years

Call during Health Centre hours to book appointment
Appointments may be in-person or virtual via phone or videoconference, depending on assessment of client need
Clients will be screened over the phone and upon arrival at the clinic

Eligibility

Four Corners Health Centre:


Residents of Malton and a member of one or more of the following groups:
  • youth ages 13-25 years
  • adults 65 years and over
  • newcomers to Canada (less than 5 years)
  • racialized communities
  • people facing mental health conditions or substance use issues
  • people who are homeless or at-risk of homelessness
  • families who are under-resourced


    Residency Requirements: Malton with postal codes starting with L4T

    Malton is bounded by Highway 427 and Finch Avenue (border with Toronto) to the east, Brampton city boundary and Steeles Avenue to the north, Airport Road to the west, and the CN rail line and Toronto Pearson International Airport to the south
