Community Health Centres of Northumberland

Provides primary health care, health promotion and community development services. Practitioners include physicians, nurse practitioners, diabetes educators, community dietitian, dentist, dental hygienist, health promoter, counsellor and community outreach worker.

  • Primary Care Outreach - Physicians and Nurse Practitioners assess and treat non-life-threatening injuries or illnesses
  • Geriatric Assessment Intervention Network (GAIN) - A network of coordinated healthcare services and partners working together to strengthen the supports and capacity of frail older adults.
  • Memory Clinic (MINT) - team approach for the early diagnosis, treatment and support of problems associated with memory loss.
  • Diabetes Education Programs - One-on-one education and supports, and groups for people living with diabetes
  • Counselling Services - Mental Health and Addiction counsellors offer individual therapy and group sessions
  • Dental Services - subsidized basic dental services for uninsured persons - incudes teeth cleaning, check-ups, tooth removal and fillings

None - for primary health services ;
A limited number of programs charge a small fee to cover unfunded costs such as food for cooking classes, yoga instructors course manuals etc., however, these can be subsidized or waived based on ability to pay.
There is a co-payment fee for Basic dental care service - potential for additional financial assistance.
