Grand Bend Area Community Health Centre

Provides primary healthcare and interprofessional health services to Grand Bend and surrounding areas. The primary care team consists of family physicians and nurse practitioners supported by registered practical nurses, a system navigator, and medical office assistants. Team-based care is provided through onsite social workers, physiotherapists, occupational therapists, registered kinesiologists, registered respiratory therapists, registered dietitians, a diabetes education team, and on-site specialists.

Fees

There are no fees for primary care with a valid Ontario Health Card. Some fees apply e.g., commercial driver medical reports. US residents and foreign visitors will be asked to pay cash for their visit.
