Lambton Elderly Outreach - Crisis Intervention and Assistance
Case managers assist and support seniors in need to work through crisis situations by offering counselling, intervention and referral information. The focus of the program is on assisting clients in crisis and abuse situations; as well as, on Transitional Care Planning for Long Term Care residents returning to the community.
Case Managers are also trained in dealing with clients suffering from abuse.
