Lambton Elderly Outreach - Retirement Home Support Program
Assists low income, at-risk seniors find supportive housing in a retirement home. Organization contracts with participating retirement homes to provide subsidy to those that qualify.
Application
A phone intake, including a financial and medical assessment will be completed
Eligibility
Applicants must meet the following conditions:
Be a Lambton Senior
Meeting the financial threshold
Not require Long Term Care
Complete an application
Agree to placement
Complete an application for suitability with one of the partner retirement homes

Fully Accessible ; Designated parking at entrance ; Wheelchair accessible automatic main entrance ; Barrier-free washroom including door and stall
