- Must be rostered with a Physician/Nurse Practitioner of Misiway Milopemahtesewin Health Centre to receive services
- Registration for a Primary Care Provider (Physician or Nurse Practitioner) must be done with Health Care Connect Ontario
Eligibility
- Must be a patient of the Health Centre
- Must be Indigenous, Metis or Inuit for Primary Care
- Must provide proof of valid provincial health care coverage
