  • Offers access to primary care through a multidisciplinary team of Family Physicians, Nurse Practitioners, a Clinic Nurse, and Mental Health Therapist
  • Manages a diabetes wellness centre
Offers primary health care including but not limited to:
  • Disease and illness cure, management and prevention
  • Education and outreach
  • Health promotion
  • Wellness activities
Clinics, Programs and Services include but are not limited to:
  • Blood sugar screening program
  • Cancer screening tests
  • Care coordination for Elders
  • Chronic disease follow-up
  • Collaboration with specialist services
  • Diabetic Educator
  • Dietitian
  • Foot care clinics
  • Health incident management (infections, bronchitis etc)
  • Immunization clinics
  • Mental health/addictions counselling
  • Prenatal care
  • Well-baby assessments

Application

  • Must be rostered with a Physician/Nurse Practitioner of Misiway Milopemahtesewin Health Centre to receive services
  • Registration for a Primary Care Provider (Physician or Nurse Practitioner) must be done with Health Care Connect Ontario

Eligibility

  • Must be a patient of the Health Centre
  • Must be Indigenous, Metis or Inuit for Primary Care
  • Must provide proof of valid provincial health care coverage
