Application
Mary Sheffield House Assessment Eligibility Form must be completed by a shelter/referral worker; call to receive the empty form; referral worker will receive a response after the intake worker reviews the completed application
Applicants are welcome to intake with their pet, the pet must be present during the intake

Eligibility
Women (both cis and trans) 55 years and older and are vulnerable due to complex physical, mental health, developmental and/or substance-use related issues
Interested in moving to permanent housing within 12 months, experiencing homelessness and staying at an emergency shelter
Completed Mary Sheffield House Assessment Eligibility Form
has income (OW/ODSP/CPP/Employment)
Able to pay the program occupancy fee
