West Parry Sound Health Centre - Community Paramedicine - In-Home Program
Paramedics provide in-home services to help vulnerable adults by partnering with healthcare providers and community organizations, striving to reduce hospital admissions, improve patient outcomes, and support individuals in managing their health conditions from the comfort of their homes.
Services:
- Seven day a week non-emergency assessments and/or treatment of minor conditions
- clinical interventions
- remote patient monitoring for blood pressure, heart rate, oxygen saturation, blood glucose, weight, and/or temperature
- facilitation of virtual care appointments
- immunization and routine medication injections
- at-home laboratory services
